Insurance Responsibility
Clients are responsible for confirming their nutrition benefits, including coverage, diagnosis requirements, and whether a referral is needed. Coverage varies by plan and carrier.

Important Coverage Notes

• All Humana plans and Medicare cover nutrition services only for a diagnosis of Diabetes or Chronic Kidney Disease (CKD)

• A referral is required for Humana and Medicare before your appointment can be scheduled

• Appointments booked without the appropriate diagnosis or referral may not be covered

Other Insurance Plans
Coverage for other insurance plans varies. To verify benefits:

• Insurance information must be submitted at the time of booking

• Benefits will be reviewed prior to your appointment

You will be notified if services are not covered or if self-pay options apply

• Insurance plans other than Humana, Aetna, Oscar or Medicare: when you click on the link below you will be redirected to Berry Street, they will take care of the insurance verification. 

Credit Card on File
A credit card may be required to hold your appointment time, but your card will not be charged unless:

• Insurance denies coverage, or

• You choose a self-pay option or

• a NO SHOW fee of $50.00
